The short answer
Wine probably gives you a headache from a mix of the alcohol itself, a grape-skin compound called quercetin that can stall acetaldehyde breakdown[3], and histamine or tyramine if your body clears them poorly[4]. Sulfites are the usual scapegoat but rarely the cause.
The sulfite myth
The first thing most people ask is why does wine give me a headache, and the first answer they reach for is sulfites. The label "contains sulfites" on every US wine bottle made them an easy target. The chemistry does not back it up.
White and sweet wines generally carry more added sulfite than dry reds, yet red wine produces far more headache complaints. Sulfite reactions are real, but they show up mostly as breathing symptoms. In a controlled wine challenge of sulfite-sensitive asthmatics, only 4 of 24 reacted to high-sulfite wine, and the authors concluded the role of sulfites had been overestimated[1].
Cleveland Clinic lists sulfite sensitivity symptoms as wheezing, chest tightness, coughing, and hives, almost all respiratory, and notes that only about 4 to 5 percent of people with asthma react to sulfites at all[2]. Headache is not on that list. If your wine reaction is head pain rather than wheezing, sulfites are the wrong suspect.
The quercetin and acetaldehyde mechanism
The most interesting recent lead is a compound in grape skins. When you drink red wine, quercetin is converted in the body to quercetin-3-glucuronide, which appears to inhibit the enzyme ALDH2 that breaks down acetaldehyde[3].
Acetaldehyde is the toxic intermediate your body makes when it processes alcohol. It is the same molecule behind the "Asian flush" reaction. When ALDH2 is blocked, acetaldehyde builds up, and high acetaldehyde causes flushing, a warm feeling, nausea, and headache[3].
This is a hypothesis, not settled fact. The lab work showed the inhibition; the researchers explicitly noted that human testing is still needed. But it fits the pattern many people report: a headache from one or two glasses of red, often fast and with flushing. Red wine carries roughly ten times the flavanol content of white because of skin contact, which is why red is the more common trigger.
Histamine and tyramine
Wine also contains histamine and tyramine, two biogenic amines that form during fermentation and sit higher in red than in white.
Histamine matters most if you break it down slowly. The enzyme diamine oxidase (DAO) clears dietary histamine in the gut, and when DAO activity is low (from genetics, certain medications, or gut conditions) histamine accumulates and can trigger headache, flushing, and a runny nose[4]. Tyramine is the other amine often blamed, the same one linked to aged cheese and cured meat. Here the evidence is weaker. A review of controlled oral challenge studies found that tyramine did not reliably trigger migraines in susceptible people, and that the histamine-in-wine link was also not confirmed under rigorous testing[5]. So amine sensitivity is plausible and worth testing for as an individual, but it is not a guaranteed across-the-board cause. If fermented and aged foods also set you off, that points toward histamine intolerance as your pattern.
Alcohol, dehydration, and tannins
Some of the headache is just the alcohol. Ethanol blunts vasopressin, the hormone that tells your kidneys to hold onto water, so you lose more fluid than you take in. A controlled study found alcohol prolonged this effect and produced measurable extra volume loss followed by counterregulation[6]. Dehydration and the electrolyte shifts that come with it are a standard part of the hangover headache, and they scale with how much you drink.
Tannins, the drying compounds from skins, seeds, and oak, get blamed too. The direct evidence that tannins cause headaches in people is thin, so treat this one as possible but unproven. If strong black tea (also high in tannin) gives you no trouble, tannins are probably not your issue.

Pattern beats theory here. Track a few variables across several occasions: the wine style, how much you drank, what you ate, whether you hydrated, and when the headache started.
Fast headache from one glass of red, with flushing, leans toward the acetaldehyde or histamine route. Headache only after several glasses and overnight leans toward dehydration.
